Release of Lab Results Authorization

Why this matters

ReachWell Health, LLC (“ReachWell”) provides access to clinical-grade diagnostic testing and delivers results through our platform.

In order for your test results to be made available to you, ReachWell must receive those results from our independent laboratory partners and present them securely within your account.

This Authorization allows that process to occur.

Authorization

Effective Jan 1, 2026

I authorize ReachWell Health, LLC, and its affiliated entities (“ReachWell”), to receive my laboratory test results from its accredited laboratory partners and make those results available to me through the ReachWell platform.

What this includes

This Authorization applies to:

• Laboratory test orders initiated through ReachWell
  
• All resulting laboratory data, including biomarker measurements and associated reports
  
• Any supporting information required to process, deliver, and display results
  
  

ReachWell will use this information solely to:

• Deliver results to me through my secure account
  
• Enable access, visualization, and tracking of my data over time
• Support platform functionality and user experience

How your results are delivered

• All testing is performed by independent, accredited laboratory partners
  
• Results are transmitted securely to ReachWell
  
• Results are displayed through the ReachWell platform for easy access and understanding
  

ReachWell does not alter laboratory results.

Important Information

By agreeing to this Authorization, I understand that:

• No medical services: ReachWell is not a medical provider and does not diagnose, treat, or provide medical advice
  
• Use of results: The information provided is for informational purposes only and is intended to support my understanding of my health data
  
• Consult a provider: I should consult a licensed healthcare provider for interpretation, diagnosis, or medical decisions
  
• Data handling: My information will be handled in accordance with ReachWell’s Privacy Policy
  
• Right to revoke: I may revoke this Authorization at any time by submitting a written request to: Privacy@ReachWell.Health. Revocation will not apply to results already processed or delivered
• ‍No impact on access: If I do not provide this Authorization, ReachWell may not be able to deliver my test results through its platform

Expiration

Unless revoked earlier, this Authorization will remain in effect for ten (10) years from the date of acceptance.

Acknowledgment

I confirm that I have read and understand this Authorization and agree to the release of my laboratory results to ReachWell for the purposes described above.
